The University of South Alabama has implemented an electronic method for submission and review of all IACUC protocols and amendments. All submissions regardless of type shall be handled through the IRBNet portal/management system.
The below descriptions provide an overview of the materials required by the type of submission. For specific instructions, please see the Tools page. For questions or concerns, please contact the IACUC office via the below methods:
Each protocol is the combination of two forms. Let the examples below provide guidance on the content expected by the IACUC when answering questions on the required forms.
USA Protocol Handbook
If your protocol was Approved prior to IRBNet then all you need to do for an Amendment is, follow the procedure for new protocols, but the IBC & IACUC Amendment Form and IACUC & IBC Application (On-Line Document) will be the only documents needed in the package.
Amendments or modifications include clarifications, requested modifications, revisions, or alterations of any kind to a previously reviewed protocol.
Through IRBNet, the PI should select the project from My Projects the project which requires amendment or modification.. Then utilize the Forms Library, On-line Document tool, and the Designer to assemble the protocol with all appropriate documentation. If a change is to protocol associates, then Link Training. Training may also have to be updated and linked if it has expired since last review. Lastly, the package must be Signed, and it must be Submitted.
Any changes to existing documents should be accessed via the Editing tool, DO NOT just attach a new form. In addition to the changes to documents themselves please also provide the following:
Amendments: IACUC & IBC Amendment Form
Modification by request: A follow up to the modification request memo.
New protocols specifically exclude modifications, amendments, clarifications, etc.. A protocol is considered new only
when it has not been reviewed by committee and isn't a change to a previously reviewed protocol.
Through IRBNet, the PI should Create New Project. Then utilize the Forms Library, On-line Document tool, and the Designer to assemble the protocol with all appropriate documentation. Then Link Training. Lastly, the package must be Signed, and it must be Submitted.
