New Hires - Temporary Faculty and Staff Benefit Orientation Forms

Below are the required new hire forms you will need to complete. Please return completed forms to Human Resources via email or in person. All forms, except Direct Deposit and Federal and State Tax forms (must submit originals), may be returned electronically via email to hrmaincampus@southalabama.edu.  An electronic signature is acceptable for those forms submitted electronically, however, you will need to first download the form, complete, sign and date, save and then submit as an attachment to the email above.
 
For the protection of your personal information, you must transmit all completed forms from your southalabama.edu email address to the email address provided above. If you are transmitting the completed forms from a personal email address, please password protect the attachment and advise HR of the password.
 
By using an electronic signature, you are agreeing that your electronic signature is the legal equivalent of your manual signature.
